Service Description
Description
South East Community Living assists people with mental health conditions to live well in the community. The program has a strong focus on identifying and addressing barriers to self management. Services are provided to people living in the Limestone Coast and are home and community based.
Referral Criteria
Referrals can be made by Housing SA, Integrated Mental Health Team (South East) Country Health SA Local Health Network or South East Community Living. Criteria includes: willingness to engage with support, diagnosed mental health condition, in receipt of a Centrelink benefit and living within the Limestone Coast.
Co-payment
